Please submit information below to secure your space on the wall of heroes and for radio submission.

there will be a one time processing fee of $15 for every video / audio submission. this will take care of the production cost and shipping of the video. once the video is completed a copy of the final product will be sent to the requesting party on a custom flash drive as well as the link to the video on You tube.

a portion of proceeds from every video/ audio submission will be donated to support veteran service organizations

* My Submission serves as permission granted to Christian Warrior Radio Network LLC to use my likeness, voice, and story in a photograph, video, or other digital media in any and all of its publications, including web-based publications, without payment or other consideration.

* I hereby authorize Christian Warrior Radio Network LLC to edit, alter, copy, exhibit, publish, or distribute my likeness, voice, and story for any lawful purpose. Additionally, I waive any right to royalties or other compensation arising or related to the use of my likeness, voice, and story.

* I hereby hold harmless, release, and forever discharge Christian Warrior Radio Network LLC from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

* I HAVE READ AND UNDERSTAND THE ABOVE RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE, I AM THE SUBJECT OF THE AUTHORIZATION OF HAVE THE AUTHORITY TO REPRESENT THE SUBJECT OF THE AUTHORIZATION.
